South East Palliative and End of Life Care (PEoLC) Network is delighted to invite you to our first clinical forum to be held on Thursday 29 July 2021, 2:30pm-4pm (online).
Our PEoLC Clinical Forum will bring together multi-disciplinary professionals, from frontline staff to clinical leaders working in palliative and end of life care in the South East Integrated Systems (ICS). This unique collaborative interface will have clinically led agenda with sole focus of improving patient care outcomes, inter-professional support, regardless of roles or settings, by speedily sharing best practice and difficult but common challenges.
The forum invites individuals to engage in rich discussions, identify areas of focus, build a shared learning agenda, disseminate best practice and develop opportunities to collaborate. Please share with all team members and reserve the date- don’t miss this unique opportunity.
Aims of the forum:
- Provide a safe space for multi-disciplinary professionals working in palliative and end of life care to share challenges, concerns and ideas
- Provide opportunities for individuals to share learning, experience and develop new knowledge and resources
- Troubleshot together to find solutions to common problems
- Enable peers to connect across organisational and professional boundaries
- Ultimately share, learn and implement what really matters in P&EOLC the very next day
Who should attend?
The forum is open to all staff working in palliative and end of life care in the South East. This includes colleagues from public, independent and voluntary sectors.
Please note, topics and agenda are currently in development, however, we would encourage colleagues that are interested in attending to sign-up via Eventbrite to stay updated with the forum development.
Here are some examples of what we hope to cover (and many more that you will suggest in the future):
- What tools, processes would enable early identification and management of sustainable P&EOLC across our systems?
- Advanced Care Plan/ReSPECT/Treatment Escalation Plans: What could support maximal impact for patient, professionals and system wide sharing in & out of hours?
- How do we address the Health Inequality in P&EOLC across our systems?
- How do we meet the National Ambitions for P&EOLC working together for our patients and with our professionals?
- How do we address the P&EOLC Focus of improving Access, Quality and Sustainability?
- What data and digital enablement focus would enable optimal care and QI?
- How do we enhance our workforce capacity, capabilities and resilience across our systems in P&EOLC?
- What P&EOLC lessons can we share and learn from each other via patient, professional and procedural feedback?
- What can we collectively learn from key P&EOLC evidence base and translate into practice speedily?
