The purpose of this draft discussion document is to provide a unified approach to the recording and sharing of Palliative and End of Life Care information throughout Hampshire & Isle of Wight (H&IOW) by combining and digitising the nationally recognised ReSPECT form with the local existing Future Planning electronic template. This paper was commissioned by the STP from the Wessex Palliative and End of Life Care Clinical Network (2016-2018).
