This is one of a set of case for Change for Diabetes that was produced by the Thames Valley SCN for the TV CCGs in 2017. Whilst its acknowledged that the data is now old, the structure, format and opportunities for improvement using this approach are not to be underestimated. The CCG leads used their Case for Change in various ways including to raise the profile of diabetes with their exec board, develop their strategy and shape their improvement programme for diabetes care.

The structure of the Case for Change can be applied to other clinical areas.
